Icon Commander 1.10
Icon Commander converts images of any format into Windows icons. You can find many images on the Internet and convert them to icons for use on your desktop, in applications and much more. Icon Commander have rich collection of icon styles.
With IconCommander you can create Windows Icon with a single action. It supports Windows Vista 256×256 icons with alpha channel.
This appears to be a really neat little program! And the reviews are mostly positive. Click the screen shot to the right to see all the options and sizes you get for an icon.

Hey this is fun! Besides as an icon you can save the images as .png and in various sizes and templates which are provided.
